Hello thr
M new to this n wud appreciate valuable help I get thru this forum.
In the medical form there's a Q's on blood condition (including thalassemia)
Now technically thalassemia means being Major, But me n my husband r both thalassemia MINOR so technically we shud tick No in the

Can you please clarify in which section of Form 80 they ask about thalassemia? From my memory, I don't remember any such question.

Sorry my mistake...it's not in the form but while filling the medical form which u do so separately,there they ask whether u have any blood condition (including thalassemia)
Now thalassemia in itself means being a Major But me n my hubby r Thal minor so we can tick a No for that Q's right?

Well, again, you have to be more precise to let us know which form you are talking about. From my memory, the mandatory forms (like Form 80 or Form 1221) does not ask any such question. So, you must be talking about some other form which you need to clarify. Is it some form to submit with you application or some form you need to fill up at the hospital when you go for your medical check up?

Also, in either case, you should consult with a doctor may be. Because, in my opinion, major or minor, if you have thalassemia, then you have it (as they didn't ask for "major thalassemia"). But ofcourse, I am not a doctor and no idea about this particular condition.

11.Have you ever had any previous passports?
Answer : YES
Because, my previous/old passport I held had all the pages stamped(but valid until 2018) and no more than 3 pages were left in the passport to travel. So I renewed and got a new passport.
12: what happened to this document? options Expired/Lost/stolen/other
what should I answer?

As it clearly says "Any maiden or Tribal name". So if your wife has any, then simply write that. If not, then leave it blank or write N/A.

In my opinion, maiden name is for someone who's an older single lady, so that's a cross to your situation. While tribal names are old school, no one uses that anymore. I would suggest type N/A or maybe if you don't want to type N/A just use X name of your wife.
