I am planing to submit Visa application with all the required details like health assessment and PCC verification on one shot.
1. When can we submit the Visa application after completing Medical Assessment?
2. Will it take time to reflect the health assessment to linked HAP ID after completing health verification?
3. Is there any way i can verify that the hospital submitted by health assessment with HAP ID

You can submit your visa application even prior to medical assessment. Medical is required but not necessarily needs to be submitted at the time of visa application. It is also recommended that you wait for CO communication for medical and PCC to avoid expiry of these documents in case if he is taking a lot of time. Once you lodge, you can check the status of your application including medical assessment submission by hospital
