ACS ACS skill reassessment

my ACS skill assessment was filled two years ago by my consultant in india. I got the assessment from ACS also. I didn't know the login details to that.Now the skill assessment has expired, i want to get my ACS skill assessment done again . This time i want to do it on my own. I have terminated the contract with my old consultancy. During filling the ACS form online, i have to enter my passport number. After which i unable to go further because it says that this account is already in our system. So i have to login first with the consultancy login credentials i guess. But the consultancy agency is not ready to give the login credentials. How to go ahead in this case

Send an email to ACS and clarify the situation, ask them for the further steps

Below are the document required in ACS
Required Documents:
1. Passport – Applicant details page only, not full passport. Ensure that the name used in your application matches the name on your passport
2. Evidence of change of name (if applicable)
3. Degree or Award Certificate • Title of Degree or Award • Name of University or Awarding Institution • Date the Degree or Award was completed – the qualification will be assessed as NOT suitable if there is no documented evidence that the degree has been completed and awarded.
4. Degree or Award Transcript • Unit or Subject Names and Grades or Marks Achieved (uploaded in chronological order) • If you have a Post Graduate degree please provide documents for the undergraduate qualification.
5. Employment References
6. Evidence of Paid employment
7. If you apply for a Recognition of Prior Learning (RPL) application, you will need to submit an ACS Recognition of Prior Learning (RPL) Form.
8. Resume/Curriculum Vitae – upload in your personal documents section
9. Migration agent authorisation form (if applicable)

In employment reference, the declarant (my manager) should also provide one of these as per section 7.4-
Employment certificate/statement of service on official company letterhead •
Leaving certificate if no longer working at the same company

My HR is not providing the above detail due to their policy. Is there any alternative document we would provide.
Please suggest.

Ask some trusted colleague to give you statutory declaration

Can we have bonafide letter inplace of "Employment certificate/statement of service".

Maybe, if it's provided by the company in their letterhead.
